I understand that the membership fee is nonrefundable. I understand that this is a basic membership only and that all other programs, including co-op, require a separate registration form and may involve additional fees. I have read and agree to abide by the Livingston Choice Learning Core Values I do hereby certify that I and my family are covered by our own medical insurance policy. I understand and agree that neither Livingston Choice Learning, nor the facilities in which group activities are held, shall be liable for personal injuries incurred by me or my children while participating in a Livingston Choice Learning event or activity. I also agree not to hold any staff, instructors, coaches, field trip coordinators, or any other person(s) liable for any personal injuries incurred by me or my children while participating in Livingston Choice Learning events and activities. I will not hold Livingston Choice Learning liable for any contagious illnesses my family may contract while participating in LCL sponsored activities.
